What steps do I follow if I have a work related injury?

You must follow the steps below if you have a work related injury:

• Seek medical help IMMEDIATELY at current facility.
• During the day go to Employee Health Services, at the facility where your injury occurred. During off hours go to the Emergency Department (ED). Identify yourself as a Consortium employee and state that your injury is work related. DO NOT give anyone in the ED your Blue Cross and Blue Shield insurance card. SHOW YOUR PURPLE CARD.
• Leave a message at the Consortium office at 860-676-1110, with your name, telephone number and date of incident. Please speak slowly and clearly.

If you receive a bill from the hospital, please call the Consortium immediately. If you fail to report the incident properly, you may be responsible for the bill.
