You will sign up for CPMRS at orientation.

• CPMRS stands for CT Prescription Monitoring and Reporting System.
• You will need to be registered for this if you prescribe controlled substances.
• The purpose of this system is to present a complete picture of a patient’s controlled substance use, including prescriptions by other providers, so that each provider can properly manage the patient’s treatment.
• Failure to comply with this requirement could result in an inability to prescribe medications and may also result in a fine.

If you are coming to orientation

You will have the opportunity to sign up for CPMRS in one of the sessions. We will also have a notary on hand to notarize the application for you, as required. We will mail the application in for you.

If you are not coming to orientation

You will need to sign up for CPMRS on your own by visiting the site https://connecticut.pmpaware.net/login. You will need your NPI number and Controlled Substance number to fill out the application. When prompted for a DEA number, please use the John Dempsey Hospital DEA number of AJ 62174616.

Once you complete the application, you will need to print it and have it notarized. View the list of notaries.
