Training Co-Sponsorship Application

If you have any cardiovascular risk factors for developing heart disease and would like to be proactive in developing a plan to reduce your risk, the Lifestyle Medicine Program may be right for you.

Cardiovascular Risk Factors

You may benefit from the Lifestyle Medicine Program if you have any of the following conditions or risk factors:

  • High blood pressure
  • High cholesterol
  • Diabetes
  • Smoking
  • Cardiac history
  • Overweight
  • Family history of cardiac disease
  • Inactivity
  • Stress

Personalized Assessment and Plan

The program is very personalized. An individual’s specific risk factors, behaviors, and limitations are evaluated, and a lifestyle modification plan is developed.

The initial visit includes a review of previous lab work and a cardiovascular risk assessment. Additionally, an analysis of an individual’s current diet, as well as their exercise and activity routine, and medical history are obtained.

Based on the initial visit, the following services may be offered:

  •  Nutrition counseling
  • Smoking cessation program
  • Weight loss plan
  • Exercise prescription
  • Stress management
