It is the program’s expectation of the fellow that the fellow will complete all required evaluations in a timely manner, and that all other paperwork be completed and/or turned-in in a timely manner as well.

All evaluations, except patient/family evaluations, are now completed online through Myevaluations.com. This system helps us to manage, collate, and analyze the information gathered.  Myevaluations.com will provide necessary feedback about the educational program and its participants.

To help monitor progress, we will evaluate Fellows in several ways.

Formal tools to evaluate the fellow are:

• Attending evaluation of competencies
• Fellow self-evaluation
• Patient Satisfaction Questionnaire
• 360 Evaluation of Fellow by staff (mid-level, nurses, Mas)
• Milestone Evaluation of Fellow

In addition to the fellow being evaluated, the fellow has the opportunity to evaluate:

• Program
• Faculty
• Rotations
