We encourage you to read more about the clinical services we provide to help you identify the clinicians who specialize in the area of neurology most appropriate to your problem. Please obtain a medical referral from your primary physician (even if it is not required by your health insurance carrier) so your doctor can provide important background information. When you have obtained a referral, please call 1-84-GET-UCONN (1-844-388-2666) to schedule your appointment.

Before you come to clinic, we will need your pertinent medical records and copies of test results to be faxed to our clinic at 860-679-0131. For brain and spine imaging studies, we need the actual images (on CD) of any imaging studies you may have had, including CT scans, MRIs, angiograms, etc., which you should bring by hand on the day of your appointment. Imaging studies done at UConn Health will be available to us through our medical records.

Neurology Clinic

The Neurology Clinic is located on the 3rd floor of the Outpatient Pavilion on the Farmington Campus of UConn Health, 263 Farmington Avenue, Farmington, CT 06030-5357.

Patient drop off/pick up and valet parking are located in front of the building. For self-parking, use Garage 1, across from the Outpatient Pavilion. A sky bridge is located on parking level 3 that connects to the building. View maps and directions.

Academic Offices

The Academic Offices of the Department of Neurology at UConn Health are located on the 3rd floor of the Administrative Services Building, on the Farmington Campus. Office hours are 9 a.m. through 5 p.m. on weekdays.

Locations

UConn Health Neurology is located on the main UConn Health campus at 263 Farmington Avenue, Farmington CT.

Botulinum toxin injections are currently performed in Neurology Clinic on the 3rd floor in the Outpatient Pavilion.

Outpatient EEG, EMG, and autonomic testing are performed on the Neurodiagnostic Floor (3rd floor) in the Main Building on UConn Health’s Upper Campus. View map and directions.
