Percent Hospital Readmissions for Ambulatory Care Sensitive Conditions

The information below presents hospital readmission rates per 100,000 for ACSCs by insurance type for years 2012 through 2017. Hospital readmission rates for ACSCs are greater for individuals with Medicare and Medicaid coverage than those with private insurance, no insurance and other insurance. The average rate of hospital readmissions for overall ACSCs from 2012 to 2017 was 17.1% for Medicare and Medicaid, 11.8% for Private insurance, 11.7% for other insurance, and 9.0% for no insurance.
In the years prior to SIM (2012-2015), the overall hospital readmission rates increased the most for those with no insurance from 9.2% in 2012 to 12.2% in 2015, and decreased the most for those with private insurance from 12.6% in 2012 to 11.3% in 2015. After the initiation of SIM, those with no insurance had the largest decrease in the overall hospital readmission rates for ACSCs from 7.9% in 2016 to 6.3% in 2017, and acute ASCSs from 9.6% in 2016 to 5.1% in 2017.
