The information below presents hospital readmission rates per 100,000 for ACSCs by CT county of residence for years 2012 through 2017. In general, New Haven had higher rates of hospital readmissions for overall ACSCs. Tolland and Windham were most likely to have the lowest rates of hospital readmissions for overall ACSCs. In the years prior to SIM (2013-2015) and after the implementation of SIM (2016-2017), the hospital readmissions rate was fairly stable.
