Percent Hospital Readmissions for Ambulatory Care Sensitive Conditions by Age
Summary of Results
The information below presents hospital readmissions rates per 100,000 for ACSCs by age groups for years 2012 through 2017. In general, hospital readmissions increased with age. The average rate of hospital readmissions for overall ACSCs was lowest for the 18 to 24 age group (12.95) and greatest for the 65 to 74 age group. This pattern was consistent for chronic and acute.
In the years prior to SIM (2012-2015) the rate of hospital readmissions per 100,000 was primarily on an upward trend with a few exceptions noted below. Since the implementation of SIM in 2016, in general, older age groups (i.e., 55 to 64. 65 to 74, and 75 and older) continued on an upward trend, while younger age groups saw a reduction in readmission rates.
