Percent Hospital Readmissions for Ambulatory Care Sensitive Conditions by Age
The information below presents hospital readmissions rates per 100,000 for ACSCs by age groups for years 2012 through 2017. In general, hospital readmissions increase with age. The average rate of hospital readmissions for overall ACSCs was lowest for 18-24 year olds (12.95) and greatest for the 65-74 age group. This pattern is consistent for chronic and acute.
In the years prior to SIM (2012-2015) the rate of hospital readmissions per 100,000 was primarily on an upward trend with a few exceptions noted below. Since the implementation of SIM in 2016, in general, older age groups (i.e., 55-64. 65-74 and 75 and older) have continued the upward trend while the younger age groups have seen a reduction in rates.
