Community Led Event Proposal Form

The UConn Health Disparities Institute’s Menopause Equity Initiative partners with community organizations across Connecticut (The Menopause Equity Collective) to raise awareness, support healing, and expand education about menopause. We encourage community members and organizations to host their own menopause equity events, and we are here to support you.

How to Request Event Support

Use this form to request HDI partnership and free resources for your community-led event. Our goal is to help make your event successful, culturally relevant, and accessible.

Who Is Eligible to Apply?

We welcome requests from community organizations, advocacy groups, faith-based groups, healthcare partners, businesses, and individuals committed to menopause awareness and education. We especially encourage applications from those serving underserved or historically excluded communities, and those who share HDI’s mission, vision, and values.

Events may be in-person, virtual, or hybrid.

  • Thought partnership and event planning support from HDI
  • Speaker suggestions and referrals
  • Information about Continuing Medical Education (CME) credits for healthcare professionals
  • Event promotion through HDI and partner social media, website, and email lists
  • Tabling at in-person or hybrid events, depending on capacity
  • Additional support tailored to your needs

As a condition of this partnership and support, we ask that:

  • HDI is listed as a partner on all promotional materials and event signage.
  • HDI and statewide partners are tagged on social media posts related to your event (if applicable).
  • All promotional materials and event signage are shared with HDI for review before public distribution.
  • HDI’s logo is included on all promotional materials.
  • HDI and statewide partner organizations have the option to table at in-person or hybrid events, if capacity allows.
  • Partner organizations may receive branded swag or health education materials to distribute.
  • HDI and statewide partner organizations may help promote your event, as needed.
