I grant permission to UConn Health Disparities Institute (HDI), its employees and agents, to tape and use audio/video images of me. Audio/video images (A/V) are any type of recording, including but not limited to photographs, digital images, drawings, renderings, voices, sounds, video recordings, audio clips, or accompanying written descriptions.
I agree that HDI owns the A/V and all rights related to the A/V. The A/V may be used in any manner or media (e.g., university-sponsored websites, publications, promotions, broadcasts, webcasts, advertisements, posters, and theater slides, as well as for non-university uses) without notifying me. HDI has the right:
1. To use, re-use, publish and re-publish the same in whole or in part, individually or in conjunction with other photographs, images or recordings in any medium, now known or hereafter to become known, including but not limited to print and electronic media, including HDI’s website, social media and for purposes of illustration, promotion, advertising, and trade.
2. To use my name in connection therewith if it so chooses.
I hereby waive any right to inspect or approve the finished images or any printed or electronic matter that may be used with them or to be compensated for them beyond the June 2021 State of Health Equity among Boys and Men of Color Summit titled, “Trauma, incarceration, and justice across the life course: transforming systems for behavioral health equity among boys and men of color.” I also hereby waive all rights and release UConn HDI, its employees and agents from, and shall neither sue nor bring any proceeding against UConn HDI, its employees, and agents for any claim or cause of action, whether now known or unknown, for defamation, invasion of the right to privacy, publicity or personality or any similar matter or based upon or relating to the authorized use and exploitation of any audio and videos.
