Summary of Results
The information below presents hospital readmission rates per 100,000 for ACSCs by race for years 2012 through 2017. In general, hospital readmissions for overall ACSCs was highest for Blacks and lowest for Other races. The average number of hospital readmissions for overall ACSCs from 2012 to 2017 was 19.1% for Blacks, 17.3% for Hispanics, 15.6% for Whites, and 13.4% for Other.
In the years prior to SIM (2012-2015), the rate of hospital readmissions was fairly stable for all races.
- Although Other races had the largest increase in the rate of hospital readmissions for overall and chronic ACSCs, and the largest decrease for acute ACSCs.
- After the initiation of SIM (2016-2017), the largest decreases in hospital readmission rates for ACSCs were found in acute ACSCs for all racial/ethnic categories.
- For chronic ACSC conditions, all racial/ethnic groups had increased readmission rates ranging from 4.1% for Hispanics to 11.4% for Whites.
