Follow-Up After Hospitalization for Mental Illness in CT, by Age Group (FUH NQF#0576)
The following information represents the percentage of discharges for beneficiaries who were hospitalized for treatment of selected mental illness diagnosis and who had a follow-up visit with a mental health practitioner. The follow-up visit occurred within 7 days and 30 days of the discharge. The results are based on individuals ages 6 years and older who had Medicare or private health insurance coverage.
Ages ranged from 6 years old to 64 years old for individuals with private insurance, and from 18 years old to 75+ for individuals with Medicare coverage.
In 2015, the year prior to the CT SIM implementation, among those with private insurance, individuals in the 35 to 44 year-old age group had the greatest percentage of individuals who received a follow-up visit than all other age groups (within 7 days 60.6%, within 30 days 81.3%). Individuals in the 18 to 34 age group had the lowest percentage of individuals who received a follow-up visit within 7 days (53.3%) and within 30 days (81.3%). Among those with Medicare coverage, individuals 75 years of age and older were more likely than other age groups to have a follow-up visit within 7 days (64.0%) and 30 days (82.1%). Individuals in the 18 to 34 age group with Medicare coverage were least likely to have a follow-up visit within 7 days (49.0%) and 30 days (74.0%).
In 2017, the most recent year data are available, among individuals with private insurance, those aged 55 to 64 were most likely to have a follow-up visit within 7 days (61.4%) or 30 days (80.7%). Individuals in the 18 to 34 age group were least likely to have a follow-up visit within 7 days (50.0%) or 30 days (64.6%). Among those with Medicare coverage, individuals 75 years of age and older were more likely than other age groups to have a follow-up visit within 7 days (62.2%) and 30 days (81.2%). Individuals in the 18 to 34 age group were least likely to have a follow-up within 7 days (64.6%) and 30 days (72.7%).
