The information below presents hospital readmissions rates per 100,000 for ACSCs by race for years 2012 through 2017. In general, hospital readmissions for overall ACSCs is highest for Blacks and lowest for Other races. The average number of hospital readmissions for overall ACSCs from 2012 to 2017 was 19.1 for Blacks, 17.3 for Hispanics, 15.6 for Whites, and 13.4 for Other.

In the years prior to SIM (2012-2015), the rate of hospital readmissions was fairly stable for all races.

  • Although Other races had the largest increase in the rate of hospital readmissions for overall and chronic ACSCs and the largest decrease for acute ACSCs.
  • Since SIM (2016-2017), the largest decreases in hospital readmissions rates for ACSCs were found in acute ACSCs for all racial/ethnic categories.
  • For chronic ACSC conditions, all racial/ethnic groups had increased rates ranging from 4.1 percent for Hispanics to 11.4 percent for Whites.
