A confidential, private consultation with one of our staff members will include a thorough nutrition assessment. During this time, they will gain insight into your typical food habits, medical history, and overall health. From this information, your dietitian will provide evidence-based advice to create realistic recommendations that can fit into your lifestyle with the intention of improving your quality of life. A patient referral form is required prior to registering for a workshop or scheduling an appointment. Ask your physician to complete the Patient Referral/Order Form and fax it to 860-679-8359.
