Facilities are also a frequent site where patients who have had a decline in functional status, are sent after hospitalization for serious illness. Anecdotally, patients frequently voice a desire to avoid living in a nursing home or similar facility, preferring to be in their own home. Due to the combination of prevalence of progressive, serious illness, high risk of mortality, and potential for receiving medical care that may not be aligned with their wishes, patients in long-term care facilities are likely to benefit from palliative care involvement.

Our long-term care rotation leverages the expertise of our robust Geriatrics and Internal Medicine programs. The fellow will spend time with physicians who have clinics located in three different long-term care facilities in central Connecticut, including Avon Health Center, McLean Assisted Living, Skilled Nursing & Rehab and Sgt. Going to multiple facilities will provide the fellow with exposure to a greater diversity of patients in terms of race, ethnicity, socio-economic class and underlying illnesses. Fellows will be precepted by an attending physician, often a board-certified geriatrician, seeing patients attending their scheduled clinic located inside a long-term care facility. Fellows will become familiar with the frequent reasons that prompt these visits, including periodic visits mandated by regulations, managing chronic multi-morbidity, acute illnesses (e.g. urinary tract infections, pneumonia), oversight of medications, and wound care (2). Finally, fellows will be encouraged to engage long-term care residents in goals-of-care discussions and completion of advance care planning.

Fellows will be responsible for conducting visits, completing appropriate documentation, and communicating with appropriate members of the clinical team to coordinate a plan of care for the patients. Fellows will be encouraged to interact with all members of the inter-disciplinary team, including physical and occupational therapists, social workers, and others who can help determine the patient’s likely clinical trajectory, which may inform recommendations for their medical care.

Teno JM, Gozalo P, Trivedi AN, et al. Site of Death, Place of Care, and Health Care Transitions Among US Medicare Beneficiaries, 2000-2015. The Journal of the American Medical Association. 2018.

Zweig SC, Popejoy LL, Parker-Oliver D, Meadows SE. The Physician's Role in Patients' Nursing Home Care: “She's a Very Courageous and Lovely Woman.
